Gentamycin injection, an aminoglycoside antibiotic containing gentamicin as the main component, inhibits bacterial protein synthesis to exert bactericidal effects, showing strong activity against Gram-negative bacteria (such as Escherichia coli, Pseudomonas aeruginosa) and some Gram-positive bacteria, indicated for severe infections like septicemia, abdominal infections, and urinary tract infections, often combined with β-lactam antibiotics for mixed infections, administered via intramuscular or intravenous injection at a daily dose of 160-240 mg for adults (divided 2-3 times) with a usual course of 7-14 days, but carrying significant risks of ototoxicity, nephrotoxicity, and neuromuscular blockade, contraindicated in allergic individuals, pregnant women, neonates, and severe renal dysfunction patients, requiring strict monitoring of renal function and blood drug concentrations, avoiding combination with other ototoxic/nephrotoxic drugs or neuromuscular blockers, and now increasingly restricted in clinical use due to toxicity with preference for safer alternatives in milder infections.

1. Infection Treatment

• Gram-negative bacterial infections:
  • Indicated for urinary tract infections, respiratory infections, intra-abdominal infections, and septicemia caused by pathogens.
  • Also active against Serratia and Acintobacter.
• Gram-positive bacterial infections:
  • Used in combination with β-lactam antibiotics to treat Staphylococcus aureus infections (e.g., endocarditis) or  Enterococcus infections (requires combination with ampicillin).
• Local applications:
  • Eye drops/ointments for ocular infections (e.g., conjunctivitis, keratitis).
  • Topical formulations for skin infections (e.g., burns, post-traumatic secondary infections).

2. Other Uses

• Preoperative bowel preparation: Oral administration clears aerobic gut bacteria to reduce postoperative infection risk (often combined with anaerobe-targeting drugs like metronidazole).
• Intrapleural or intraperitoneal irrigation: Local administration to control infections (under medical supervision).
